At Lighthouse Hospice, we are dedicated to upholding the rights of our patients and ensuring compliance with federal and state regulations. As part of this commitment, we provide the following notices to all participants, beneficiaries, enrollees, and applicants of our hospice programs and activities:
Hospice Conditions of Participation
In accordance with Hospice Conditions of Participation 418.52 and 418.104(a)(2), Lighthouse Hospice is required to:
– Provide a notice of patient rights.
– Obtain a signed acknowledgment from patients confirming receipt of this notice.
Federal Civil Rights Regulations
Federal civil rights regulations require Lighthouse Hospice to provide the following notices:
- Notice of Nondiscrimination
Discrimination is against the law. Lighthouse hospice with applicable federal civil right laws and does not discriminate, exclude people or treat them less favorably because of social status, political belief, race, color, religion, national origin (including limited English proficiency and primary language), sex (including sex characteristics, including intersex traits; pregnancy or related conditions; sexual orientation; gender identity and sex stereotypes), age, disability (including mental illness and substance use disorders) or any combination thereof with regard to admission, access to treatment or employment. - Notice of Availability of Language Assistance Services and Auxiliary Aids and Services
We provide people with disabilities reasonable modifications and free appropriate auxiliary aids and services to communicate effectively with us, such as qualified sign language interpreters and written information in other formats (large print, audio, accessible electronic formats, other formats). We provide free language assistance services to people whose primary language is not English, which may include qualified interpreters and information written in other languages.
